Recognition as a Diabetes Provider of Excellence

The New York State Health Foundation makes available grants of up to $2,500 per physician who achieves excellence in diabetes care as demonstrated by attaining recognition under the excellence demonstrates to patients and peers that you are a leader in diabetes care and outcomes and may qualify you for incentive payments from health plans.
