Diabetes Care Recognition Programs

Public recognition as a diabetes provider of excellence demonstrates to patients and peers that you are a leader in diabetes care and outcomes. These national recognition programs qualify you for incentive payments from health plans. For a limited time, the Diabetes Campaign will pay the application fee for achieving recognition from the following programs.

  • National Committee for Quality Assurance's (NCQA) Diabetes Recognition Program
    This program was designed by NCQA and the American Diabetes Association to recognize physicians and other clinicians who use evidence-based measures and provide excellent care to their patients with diabetes. Those who achieve recognition show their peers, patients, and others in the health care community that they are part of an elite group that has met high standards of diabetes outcomes in providing the highest-level diabetes care. Eligible providers abstract data from the charts of 25 diabetes patients and submit this information to NCQA for review.
  • Bridges to Excellence Diabetes Care Link
    Bridges to Excellence (BTE) is a not-for-profit organization developed to create significant leaps in the quality of care by recognizing and rewarding health care providers who demonstrate that they have implemented comprehensive solutions in the management of patients and deliver safe, timely, effective, efficient, equitable and patient-centered care. To achieve BTE Diabetes Care Link Recognition physicians must submit clinical data that demonstrate that they have adopted better systems of diabetes care and achieve good outcomes for patients with diabetes.
